We collect information you provide during enrollment and ongoing care management, including your name, contact information (including mobile phone number), health conditions, and communications with your care team.
SMS / Text Messaging Program
If you consent to receive text messages from your care manager, we use your mobile phone number solely to send you care coordination messages, such as appointment reminders, care manager check-ins, and health alerts related to your chronic care management program.
- No sharing of mobile information. Mobile phone numbers and SMS consent information are not sold or shared with third parties or affiliates for marketing or promotional purposes. Your phone number and SMS opt-in status are used only to deliver the care coordination messages described in this policy.
- Message frequency varies based on your care plan, typically a few messages per month.
